Role Description

As a Community Liaison, you will be responsible for building strong relationships with referral sources and generating up to 50+ qualified referrals per month . This is a high impact role for a motivated professional with a proven track record in the Home Health industry.

Requirements

• Currently working in the Home Health industry
• Proven ability to consistently deliver high-volume referrals (50+ per month)
• Strong network of healthcare contacts
• Excellent communication and relationship-building skills
• Self-motivated and goal oriented

Responsibilities

• Develop and maintain relationships with hospitals, physicians, senior living communities and other referral sources
• Execute strategies to increase patient referrals and grow census
• Represent the agency at networking events and community outreach programs
• Collaborate with internal teams to ensure smooth onboarding of new patients
